Thank you for completing the online GAP Claim Form. A representative will contact you in 1-2 business days.

Meanwhile, the following documentation will be required.

Lien Holders please provide the following:

  1. Copy of the Retail Installment Sales Contract;
  2. Kelley Blue Book Retail Report, or equivalent, at the time of sale;
  3. Complete Payment history showing the posting of the Total Loss settlement check and any refundable products.

GAP Customers please provide the following:

  1. Total Loss Settlement Statement (ACV, Taxes, Fees, Salvage, Deductible);
  2. Total Loss Vehicle Valuation Report;
  3. Police Report or Written Facts of Accident statement;
  4. Auto Policy declarations page covering date of loss;
  5. GAP requires the cancellation of any refundable products (ex. Vehicle Service Contract) as of the date and mileage of the Total Loss. Please contact your provider as soon as possible.
